Part 1. 申请专业 (This part is very important)
1. Why do you choose the major you apply for? What specific branch or concentration are you most interested in? （你研究生为什么想要读这个专业？你最感兴趣的分支/方向是什么？）专业术语请尽量用英文全称表示，不要用缩略语。
2. What is the biggest advantage and disadvantage for you to apply for this major? If you intend to change your major, please give your reasons in details. （你认为你申请本专业的最大优势和弱势是什么？弱势方面你是如何改进的？如果是转专业申请，请详述你的理由。）
Part 2. 教育背景－本科阶段
1. What have you learnt in your bachelor degree? How will it benefit your application? （你本科专业学到了什么知识或技能？对申请有什么帮助？）注：请从专业知识、研究方法、计算机软件技能等方面进行概括总结。
2. What activities did you participate in during undergraduate period? 本科期间，你进行过哪些活动？注：注明活动/实习具体年月、地点、内容、你承担的工作、你做出的贡献、取得的成果、你的工作是否有创新、你学到什么、有何感想。在简历的基础上，挑选重要的经历分类进行详细阐述
C．海外交流交换经历：
3. Did you attend the research of any experiment project during your undergraduate period? （本科期间，你参加过哪些课题的研究或者实验项目？若有，请提供实验或者研究报告摘要。） 注：详述研究或试验的目的、步骤、你承担的工作，你的工作是否有创新，你是否知道国内外此类研究或试验的进展。如果你的研究或试验已结束，你取得了什么成果，从中学到了什么、有何感想。涉及到专业术语请提供英文表述。
4. Publications, graduation thesis/design during undergraduate period. （本科期间，是否有发表论文，或者毕业论文/毕业设计？若有，请提供论文中英文题目及摘要。）
Part 3. 工作经历（仅适用于全职工作经验）
1. Please describe your full-time job position and responsibility in details. What do you gain from the job? Do you encounter any setback and how you solved it? （请详细描述你的工作内容和职责。在工作中你取得了哪些成绩？）注：请从沟通能力、领导能力、团队合作、创新精神、吃苦精神方面进行事例选择和阐述。
2. How does your full-time job benefit your further education？（你的工作经验对去美国继续学习此专业有哪些帮助？）
Part 4. 职业规划 （This part is very important）
1. Which research area would you like to further pursue during your graduate study? （研究生阶段有无具体研究计划？例如对哪个分支方向或问题领域感兴趣，打算从事相关研究。）
2. What are your specific post-graduate short-term and long-term career goals? （硕/博士毕业之后的的长短期职业目标是什么？）注：请从毕业后的第一份工作-----3-5年短期目标-------5-8年长期目标三个步骤来详细阐述。切忌空泛。
Part 5. 其他信息
1. Is there any other relative information helpful for your application? （其它任何对申请有用的相关信息？） 注：例如你的性格特征，特长（音乐、体育方面），兴趣爱好对于学习或者职业规划的促进作用等。
